The Atlantic District requires that all Players, other than Adult or Disabled Disciplines, be verified by the District Registrar prior to being approved to a USA Hockey Team Roster.  Proof of both the players birthdate and citizenship are required to complete the verification process.  Once the player is registered with USA Hockey, a Birth Certificate or another form of Government issued ID is required to be submitted for review and verification. Please provide .pdf files, not .jpeg.

This verification is not valid for Player Transfers of any type.  Player Transfers and the associated documentation must still be provided to, and processed by, USA Hockey.

In order to be released from a team, please complete the following form, print and have it signed by the President of the Association authorizing the release.  This form is to be provided to the Players new Association.

The USA Hockey Concussion Management Protocol and most state statutes require that an athlete be removed from any training, practice or game if they exhibit any signs, symptoms or behaviors consistent with a concussion or are suspected of sustaining a concussion. The player should not return to physical activity until he or she has been evaluated by a qualified medical provider who has provided written clearance to return to sports.
